The Licensed Practical Nurse:

Under the supervision of a RN and in accordance with agency policies and state practice, the Licensed Practical Nurse provides nursing services to individuals and families in their residence under the supervision of a registered nurse.

1. Performs prescribed individualized patient care to patients as delegated. 
2. Participates in patient case conferences as appropriate; is an active member of the interdisciplinary team.
3. Assists the RN in the preparation, evaluation and modification of the patient's plan of care as appropriate.
4. Observes, documents and reports to the RN as appropriate, the patient's physical and emotional needs and reactions to care.

• Must hold at least one active Licensed Practical Nurse license in either VT or NH at the time of hire. Employees must have active Licensed Practical Nurse licenses in both VT and NH within 30 days of their hire date.
• Graduate of an accredited school as a Licensed Practical Nurse
• One year of experience as an LPN in a home health care setting is preferred

• Must hold at least one active Licensed Practical Nurse license in either VT or NH at the time of hire. Employees must have active Licensed Practical Nurse licenses in both VT and NH within 30 days of their hire date.
• Valid driver’s license, and a clean driving record
• Current car insurance, which meets minimum standards
• BLS within 90 days of hire
