Provides assistance to patients/families and health care providers to identify barriers to care, provide information, and facilitate access to tangible, supportive, community, and governmental resources related to patient health care needs and social determinants of health.
• Acts as an advocate on behalf of patients/families in obtaining needed services, resources, and community supports for which they are eligible, particularly when barriers to care impact health outcomes.
• Maintains and updates resource directories, community resource information, and related educational materials and applications for distribution to staff, patients, and families.
• Documents services provided, referrals, interventions, and patient interactions in the patient’s medical record and applicable databases.
• Negotiates and advocates access for patients to skilled, acute, or long-term care facilities. Identifies available transportation resources and works with patients/families to facilitate safe and cost-effective transportation arrangements.
• Works with patients/families to assess eligibility for medication assistance, co-pay assistance, financial assistance, transportation resources, and other community-based supports.
• Facilitates patient/family applications to public and private governmental and community assistance programs, including Medicaid and other available state and federal resources.
• Provides information and assistance to patients/families to secure financial assistance, health coverage, community resources, organizational financial assistance, and other governmental programs.
• Collaborates with healthcare providers, care teams, and community agencies to identify and address barriers to care and facilitate access to appropriate resources and support services.
• Assists patients and families in navigating healthcare, community, and public assistance systems to support access to care and improve health outcomes.
• Performs other duties as required or assigned.
