Provides assessment, counseling, information and referral, advocacy services, and other resource assistance to patients directly or through consultation.

Responsibilities
Provides complete psychosocial assessments of patients and families.
Provides counseling to individuals and families to help solve social and emotional problems.
Coordinates community resources to address/meet identified needs.
Assists families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community.
Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
Organizes support groups.
Documents objective, concise and pertinent clinical data in the medical record.
Attends multidisciplinary clinics with the CF team
Conducts regular screening for depression and anxiety
Builds familiarity with and helps patients access CF-related resources
Attends national and regional CF conferences yearly along with other team members
Serve as a Mental Health Coordinator as defined by the CF foundation, helping patients locate mental health providers including therapists and psychiatrists for ongoing mental health needs
Qualifications
Master’s Degree in Social Work (MSW) required.
Social work experience in an acute, home care, or ambulatory setting preferred.
Strong organizational skills, verbal and written communications skills.
