Overview
Provide patient assessment, teaching and follow up instruction for adult patients with diabetes in the outpatient clinic setting (New London Hospital and Newport Health Center).
Responsibilities
Collaborates with primary care providers and teams to support patients with diabetes and their families/caregivers.
Serve as coordinator of the American Diabetes Association Diabetes Self-Management Education and Support (DAMES) Program. Collect required data and report as required.
Serve as a coordinator of the CDC - National Diabetes Prevention Program (National DPP). Facilitate, collect required data and report as required.
Coordinate and oversee the planning, implementation, evaluation and effectiveness of the diabetes education program in collaboration with the primary care teams.
Actively participate in quality initiatives related to care of diabetes patients.
Act as a liaison to D-HH for purpose of the ADA-DSMES program.
Other duties as assigned.
Qualifications
Bachelor's Degree in Nursing preferred
3-5 years' experience as a Certified Diabetes Educator in the ambulatory setting
