Overview

Provides the Dementia Resource Program within the Aging Resource Center process coordination for initiatives for older adults and caregivers, including needs assessment, material development, communications, education, data analysis, administration, tracking and reporting. This role provides support and resource navigation to family members and individuals acting as caregivers for individuals with dementia or aging needs, aiming to alleviate caregiver stress, improve care quality, and connect them with appropriate community resources to maintain their well being. Supports the Geriatric Center of Excellence.

 

Responsibilities

1. Participates in the analysis and design of solutions for program projects and processes of all levels of complexity, supports and collaborates with facilitators in execution and delivery of programming; assists in conducting needs analysis for initiatives of all levels. Answers and triages inquiries received by telephone, mail, or in person.
2. Provides assistance in the identification and referral to resources for the health care team and patients/families. Meets with patient/family to assess the need for services. Consults, educates, and directs the health care team to clarify the need for services. Determines the availability of services required. Provides patient/family information concerning available resources. Assists patient/family in obtaining appropriate resources. Maintains expertise in resources for older adults and their families.
3. Compiles Dementia programmatic data for inclusion in reports, presentations and other communications. Prepares reports and distributes them to Aging Resource Center members and leadership as requested.
4. Identifies opportunities to educate older adults and advocates programs and processes that support the center initiatives.
5. Develops collateral materials and analytics for programs and processes that are center specific (i.e. learning, handbook, program booklet, etc.) as appropriate.
6. Acts as a central resource to organize, obtain, produce and maintain Aging Resource Center program materials and process documents.
7. Responsible for supporting the scheduling, delivery, and evaluation for programs and processes.
8. Provides technical support for older adults as needed related to programs and systems.
9. Assists and serves as project team member for the Geriatric Center of Excellence and associated leadership.
10. Performs other duties as required or assigned.

Qualifications




  • Bachelor’s degree 2+ years’ experience in an administrative/office setting


  • Proficient in Microsoft Word, Excel and PowerPoint; Access preferred


  • Yellow Belt distinction preferred.


  • Excellent verbal, written, and presentation skills


  • Highly organized with demonstrated ability to meets deadlines in a high pressure environment


  • A self-starter, recognizing work to be done prior to being asked.


  • Flexibility and adaptability to changing needs.


  • Ability to demonstrate professionalism and maintain high degree of confidentiality required.
