Overview

The Social Worker at Hampstead Hospital provides therapeutic and case management services to patients and their families. This position collaborates closely with aftercare providers and other stakeholders to ensure a seamless continuum of care. The role conducts individual, family, and group therapy as needed, actively participates in treatment planning, and develops psychosocial formulations while coordinating with internal and external stakeholders to ensure a smooth patient transition planning.

Responsibilities

  1. Provides direct and indirect social work services to patients and their families, including group and individual counseling, case management, psychosocial assessments, aftercare planning, and facilitating family meetings.
  2. Coordinates discharge planning and continuing care plans in accordance with treatment team recommendations, policies, and standards.
  3. Provides clinical consultations to other treatment team members and plays a key role in training multidisciplinary staff. This includes educating them on program policies and procedures and evidence-based interventions for the population served.
  4. Develops and maintains professional relationships with other community healthcare providers . Provides consultation to collateral outpatient providers, residential programs, and schools.
  5. Perform level-of-care assessments and related pre-certification reviews with insurance companies.
  6. If assigned to the Admissions Department, will complete level-of-care assessments, and assist with triage tasks to include conducting phone intakes, facilitating all aspects of the referral process, and collaborating with medical staff and program directors regarding potential admissions.
  7. Completes and reviews documentation in accordance with all Joint Commission and Medicare standards applicable to Social Work. LICSWs may provide direct clinical supervision and mentoring to less experienced social workers.
  8. Promotes the hospital's culture of patient safety in a non-punitive atmosphere; reports medical errors and significant events; participates in hospital safety training programs and Performance Improvement activities.

Qualifications

  • Bachelor's degree from a recognized college or university with major study in social work, psychology, education, human services, sociology, cultural anthropology, or behavioral science.
  • OR
  • Bachelor's degree in any subject field with at least twenty-four (24) credit hours in social work, psychology, education, human services, sociology, cultural anthropology, or behavioral science.
  • Three years' experience as a Social Worker or one year as a Social Worker Trainee and two years as a Social Worker I.
  • Preferred Qualifications:
  • Preferred related field work experience and/or previous experience with patients/families in a psychiatric setting and candidates who have been aligned with NH’s System of Care, supporting services that are: family-driven, youth-guided, community-oriented, trauma-informed, and culturally and linguistically competent.

Required Licensure/Certifications

  • Licensed in Social Work or license eligible in New Hampshire.
  • CPI Certification (within 3 months of hire)
  • BLS Certification (within 30 days of hire)
  • License eligible clinicians are required to pursue LICSW licensure.
