Provides program oversight, management, and strategic direction to the department.

Responsibilities

  1. Monitors and evaluates performance of cystic fibrosis program and assists with establishment of program goals and improvements.
  2. Identifies opportunities and strives for innovative strategies to improve patient care.
  3. Works with others to develop a cystic fibrosis education plan for staff and patients/families.
  4. Implements and evaluates approved goals and objectives, standards of care and practice, policies, and procedures.
  5. Monitors cystic fibrosis guidelines, order sets, and clinical pathways.
  6. Leads the team in initiating, developing, implementing, and monitoring, and measuring quality improvement processes.
  7. Implements and evaluates processes to achieve compliance with regulatory and payer requirements.
  8. Educates patients and families on cystic fibrosis processes, return appointments, and home IV follow up.
  9. Develops process for documenting and providing feedback to all stakeholders involved in the cystic fibrosis care process.
  10. Serves as liaison to other care teams regarding mutual education, clinical care, and quality improvement issues.
  11. Works with the CF team to identify and assist with care needs for newly diagnosed patients within the scope of practice.
  12. Assists in ensuring cystic fibrosis patients are reviewed at least annually for appropriateness of care following practice guidelines.
  13. Assists, as directed, in recruiting and consenting patients for research studies.
  14. Participates in strategies for product/service standardization and utilization.
  15. Performs other duties as required or assigned.

Qualifications

  • Bachelor’s degree in Nursing, or in a related field, with 2 years of experience in cystic fibrosis care required.
  • Experience as a clinic coordinator preferred.
  • Strong leadership skills.
  • Effective interpersonal/communication skills required.
  • Self-starter with initiative.
  • Proficient with computer applications, including data entry software, word processors, and reporting platforms.
  • Ability to coordinate and prioritize multiple tasks at once required.

Required Licensure/Certifications

  • Registered Nurse in the state of New Hampshire.
  • BLS certification required.
