Overview

Quality, Safety, & Outreach Coordinator RN 

 

Acts as an internal consultant to further value through quality assurance, innovation of systems and processes, and maintains federal regulatory standards for the transplant surgery department. Partners with the transplant leadership team and Transplant Quality Program Coordinator to monitor quality metrics within the transplant team members. Develops clinical policy and procedures that meet DH, the OPTN and CMS standards of care for transplant candidates, transplant recipients and living donors. Provides outreach education to dialysis units and nephrology offices for professional staff, as well as the ESRD patients within their scope of practice.

Responsibilities

  1. Assists in the maintenance of structures and processes which support improvement and patient safety, enable the collection, storage, and analysis of improvement and safety data, and enable compliance with external regulatory agency requirements.
  2. Uses clinical knowledge in formal and informal consultation with individual staff, clinicians and managers, to address clinical quality and safety concerns.
  3. Develops, and maintains on-going policies and procedures that meet DH standards of patient care, as well as meets all regulatory policy mandates from the OPTN and CMS.
  4. Participates in the development of strategies to perform in-depth data collection and data & process analyses to identify opportunities for improvement and provide input into institutional decision making and goal setting.
  5. Participates in clinical adverse event task groups.
  6. Collaborates with the pre-transplant team to provide professional and patient education to dialysis units and nephrology offices within the referral area of the DH transplant program
  7. Develops strategies to maintain and increase patient referral patterns from the dialysis units and nephrology offices to the DH transplant program.
  8. Performs other duties as required or assigned.

Qualifications

  • Graduate from an accredited Nursing Program required.
  • Bachelor’s degree in Nursing with 4 years of clinical practice experience required.
  • Master’s degree in a business or healthcare-related field with prior health systems experience preferred.
  • Certification in modern improvement methodology (Lean, Six Sigma, etc.) and/or Professional Project Management and/or Healthcare Quality preferred.
  • Prior experience applying modern improvement methodology to lead and manage change preferred.
  • Demonstrated excellence in facilitation, teaching, and interpersonal skills as well as experience in the concepts and applications of continuous improvement are essential.
  • Local NE area travel required

Required Licensure/Certifications

  • Licensed Registered Nurse with NH eligibility
  • BLS
  • Valid driver’s license with a clean driving record required in accordance with D-H’s Driver Disqualifications
