Assists patients and their families with navigating and accessing community services and coordinating health care services to improve patient care and outcomes.
Responsibilities
Provides informal counseling, support, and follow-up; making home visits to patients, basic motivational interviewing and goal setting with patients/families.
Follows up with patients via phone, home visits and visits to other settings.
Assists patients with completing applications and registration forms.
Helps patients attend appointments as well as attend them when appropriate.
Provides referrals for services to community agencies.
Assists patients in connecting with transportation resources and provide appointment reminders.
Work closely with health care team to help ensure that patients have comprehensive and coordinated care.
Performs other duties as required or assigned.
Qualifications
High School graduate or equivalent required.
Must successfully complete Community Health Worker training provided.
Required Licensure/Certifications
Valid driver’s license with a clean driving record required in accordance with D-H’s Driver Disqualifications Guidelines.
