Overview

Works as a part of the Dartmouth Hitchcock Emergency Department and other select D-H departments to provide Recovery Coaching services to qualifying patients pre and post discharge. Uses shared experience, motivational interviewing, and practical problem solving to provide non-clinical services to qualifying D-H patients affected by substance use disorders and/or substance use crisis events. Act as liaison between patient and institutional/ or community resources to help patient engage in appropriate substance use treatment,  peer recovery, and/or harm reduction services to promote successful linkage to treatment services.  Services will be provided within the assigned department(s) with appropriate follow-up.

 

This is a full-time, 40-hour/week position. While there is flexibility in hours, our busiest times tend to be evenings, and some nights and weekends will be required.

Responsibilities

  1. Meets with patients at request of patient and/or department providers, using training and shared experience to engage patients and inform them about opportunities for support.
  2. Educates patients and/or their caregivers about treatment, recovery and harm options, and offers to assist with accessing these services post discharge.
  3. Educates patients and/or their caregivers about overdose reversal medication and works with D-H providers to dispense these medications when appropriate.
  4. Provides follow-up calls/meetings with patient within 24-48 hours of discharge, and ongoing if needed, providing referrals and continuing support for assessment, treatment, recovery, and harm reduction services.
  5. Assist individuals to identify and develop natural community supports in order to foster independence and community integration, including social determinants of health.
  6. Develops and maintains collaborative relationships with local treatment teams and in-patient treatment facilities to assist clients with treatment needs.
  7. Provides follow-up calls at scheduled intervals,  including to those who initially decline post discharge services to re-offer services and to check on well-being.
  8. Maintains professional boundaries at all times, accurately represents the role of Recovery Coach and encourages patient to seek clinical and/or medical advice from their appropriate provider.
  9. Attends meetings and/or trainings as required or encouraged by departmental leaders.
  10. Reports status of patient cases to program supervisors and department leaders as required by program.
  11. Documents program measures and outcomes in patient’s medical record or as otherwise required by program.
  12. Abides by HIPAA, 42 CFR, and any additional regulations set forth by Dartmouth- Hitchcock.
  13. Perform other duties as required or assigned.

Qualifications

  • Completion of 30-hour Recovery Coach Academy training or comparable training.
  • Completion of 16-Hour Ethical Considerations for Recovery Coaches training or comparable training.
  • Completion of Certified Recovery Support Worker training within twelve months of hire.
  • Lived experience valued; experience relating to persons with substance use disorders needed; empathic and non-judgmental approach essential.
  • Experience with, or educational background of, recovery concepts and practices of recovery that honor any pathway to recovery
  • High School diploma or equivalent.
  • Has access to dependable transportation; vehicle and license highly preferred.
  • Excellent interpersonal, communication, listening, and customer service skills.
  • Ability to develop professional working relationships with community partners and organizations
  • Developed problem-solving skills
  • Proficiency with MS Word, Excel, PowerPoint, Outlook. Medical record documentation experience a plus
  • Experience with doing both formal and informal presentations
