Overview

20 HOURS/WEEK - Kidney Transplant Department

 

The Living Organ Donor Social Worker role it is to serve as a donor's representative throughout the donor care continuum, communicate with the medical team on the donor’s behalf and be available to discuss any questions, concerns or issues that may arise at any time throughout the donor’s experience. Evaluates the potential living donor for any social barriers to donation and provides assessment, counseling, information and referral, advocacy services, and other resource assistance to donors either directly or through consultation.

Responsibilities

  1. Serves as the donors’ representative throughout the donor care continuum.
  2. Communicates on the donor's behalf with the medical team and/or obtains additional information to ensure that a donor's decision to donate.
  3. Be available to discuss any questions, concerns or issues that may arise at any time throughout the donor’s experience.
  4. Assesses components of informed consent for organ donation with full knowledge of the possible risks and benefits.
  5. Provides complete psychosocial assessments of living donors and their support system.
  6. Provides counseling as needed to help solve social and emotional problems.
  7. Coordinates community resources to address/meet identified needs.
  8. Assists donors and their family members in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community.
  9. Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
  10. Organizes support groups.
  11. Documents appropriate objective, concise and pertinent clinical data in the medical record.
  12. Performs other duties as required or assigned.

Qualifications

  • Master’s Degree in Social Work (MSW) required
  • Social work experience in an acute, home care, or ambulatory setting preferred.
  • Strong organizational skills.
  • Good verbal and written communications skills.
  • Sound decision making and judgment.
  • Time management, negotiation skills and a proven ability to deal with conflict resolution.
  • Comfortable with basic computer skills.

Required Licensure/Certifications

  • Licensed Independent Clinical Social Worker (LICSW) in New Hampshire preferred
