Responsibilities

Physical Therapists at DHHC screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.

 

The PT will:

·       Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.

·       Carry out appropriate treatment programs in accordance with agency policies and professional standards of care.

  • Communicate with all members of the health care team.
  • Document evaluations, treatment goals, and objectives.
  • Teach patients and family members the rationale for treatments and procedures.

Qualifications

REQUIRED QUALIFICATIONS

 

  • Must be a licensed Physical Therapist in Vermont and New Hampshire or hold a compact license.
  • Graduate of an accredited school in Physical Therapy

 

PREFERRED QUALIFICATIONS

 

  • One year of experience as a PT in a home health care setting is preferred

Required Licensure/Certifications

  • Unencumbered PT license in VT and NH or compact.
  • Driver’s license, minimum requirements on car insurance, and a clean driving record
